- Download the medical questionnaire here (print out page 1-5) or get it at the surgery
- Fill in your details and have it ready at your examination appointment
- Get in contact with the surgery +61 (07) 3412 7777
- Pass the examination
- Get your medical certificate
What else?
- All further information you might need, e.g. costs will be given to you when you arrange your appointment.
- Under certain circumstances specialist examinations might be required.
